Questions that are allowed to change.
These are not rhetorical questions with predetermined answers. Some may lead to useful instruments; some may already have strong answers elsewhere; some may be revised or retired.
1. What is the requirement?
How does an institution determine the amount of clinical capability required by a body of work? When is FTE a useful measure, and when does it describe the staffing arrangement rather than the underlying requirement?
2. What demand is visible?
Which forms of clinical work appear in encounters, referrals, schedules and utilization measures—and what meaningful work occurs outside them?
3. Where does the next increment go?
Is additional work absorbed, shifted, deferred, transferred, handled informally, performed after hours, declined, or prevented from becoming visible at all?
4. When is expansion warranted?
How do institutions distinguish temporary variation from a durable requirement for additional capability?
5. What work requires which capability?
What requires specialist judgment, physical presence, rapid response, continuity, local context, or another professional capability?
6. How are decisions made in practice?
What formal measures, proxies, heuristics and experienced judgments do organizations already use?
7. What can founders and operators teach us?
How do organizations decide when to add clinicians, where to place them, what to centralize, what service levels to promise, and when to refuse demand?
8. When should delivery architecture enter?
Which requirements or constraints favor employment, contracting, locums, telemedicine, fractional capacity, regional networks or hybrid models?
9. How should capacity change over time?
What evidence should cause capacity to be maintained, increased, decreased, or supplied differently?
10. What are we missing?
Which established discipline already addresses these questions better? Which variables are absent? What would cause CCI to change its mind?
